Individual Tax List

The following is a general listing of items needed to prepare an individual tax return, additional items may be necessary depending on your situation:

  • Personal and Dependent Information
    • Your social security number or tax ID number
    • Your spouse’s full name and social security number and tax ID number
    • Dates of birth and social security numbers of dependents
    • Childcare records (including providers tax ID) and education payments
  • Income
    • Form W-2
    • Form 1099-Misc (received from a person or entity you provided services to during the year)
    • Schedules K-1 (investment in partnership interests)
    • Form 1099-R (pension, IRA, annuity income)
    • Form 1099-SSA (social security)
  • Business Income and Expenses
    • Form 1099-Misc (received from a person or entity you provided services to during the year)
    • Income records for income not reported on 1099s.
    • Records of all expenses
    • Home office information
    • Asset information for depreciation
    • Business use of vehicle expenses (mileage, parking & tolls, or actual expenses)
  • Interest & Dividend Income
    • Forms 1099-INT, 1099-OID, 1099-DIV (from banks and other financial institutions)
    • Forms 1099-B & 1099-S (income from sales of stock or other property)
    • Dates of acquisition and records of your cost or other basis if basis not reported on 1099-B
  • Other Income
    • Alimony received
    • Gambling winnings
    • Prizes and awards
    • Hobby income and expenses
    • Unemployment, state tax refund (1099-G)
  • Deductions and Credits
    • Real estate and personal property tax records
    • Record of moving expenses not reimbursed by employee
    • Mortgage interest (Form 1098)
    • Amount of state/local income tax paid (other than wage withholding)
    • State and local sales tax paid
    • Invoice showing amount of vehicle sales tax paid
    • Cash amounts donated to charity (churches, schools, other charitable organizations)
    • Records on non-cash charitable donations
    • Amount paid for preparation of last year’s tax return
    • Employment related expenses (not reimbursed by employer)
    • Job hunting expense
    • Health Savings Account (HSA) contributions (Form 5498-SA)
    • Amounts paid for healthcare insurance and to doctors, dentists and hospitals.
    • Miles driven for medical or charitable purposes
    • Expenses related to investments
    • Receipts for energy-saving home improvements
  • Record of estimated tax payments made during the year
